Connect at Home™ is a therapist-guided cognitive stimulation program designed for a family member or trusted support person to deliver at home.

What Connect at Home IS

✔ A structured, evidence-based home Cognitive Stimulation Program

✔ Designed for people living with mild to moderate dementia
✔ Professionally guided by a registered physiotherapist
✔ Delivered at home by one consistent support person
✔ Includes 20 structured sessions and 8 weeks of coaching support

What Connect at Home Is NOT

✘ Not hands-on therapy delivered by us
✘ Not clinical assessment or diagnosis
✘ Not personalised treatment planning
✘ Not medical or allied health care
✘ Not suitable for rotating staff or organisational delivery

What You Receive

The Full Program Includes:

  • Printed 20-session activity manual 

  • Physical resource kit (physical and/or digial)

  • 8 weeks of structured coaching support

  • Access to a private community

  • Guided framework for delivering sessions at home

Sessions take 20–30 minutes and fit into everyday routines.​

Who This Program is Ideal For

✔ People living with mild to moderate dementia
✔ Individuals who are medically stable
✔ Those able to participate in short seated activities with supervision
✔ Situations where one consistent support person can deliver sessions

Safety & Supervision​

​To participate safely, the person must be able to sit and engage safely, not be at risk of swallowing small items, and be supervised at all times. 

If circumstances change during the program, participation should pause and professional advice sought.

Support for Carers, Too

Caring for someone with dementia can feel isolating, but you’re not alone. With Connect at Home™ also includes optional access to an online community where you can:

  • Connect with other carers who understand.

  • Share experiences and feel supported.

The program also provides enjoyable activities you can do together, supporting connection and adding structure and enjoyment to everyday life.
