Cognitive Stimulation therapy. Evidence-based therapy to support brain health
Home environmental assessment and advice
Addressing rehabilitation participation barriers.
Referral to other Allied Health Providers as needed.
Trial of walking aids and outdoor walking programs.
Connection to community recreational activities.
Tips for maintaining functional activity levels and fatigue management.
Regular review of care plan for optimal functional independence.
Education on well-being and the importance of social connections.
Dementia education/ support and lifestyle recommendations
Linking in with Dementia support organisations if desired
